1. About Us

Claradina Care Connect (3C) Services Ltd ("3C", "we", "us", "our") is a private limited company incorporated in Ghana under the Companies Act, 2019 (Act 992).

  • Company Registration Number: CS088800426
  • Taxpayer Identification Number (TIN): C0067227759
  • Registered office: House Number EF M79/6, Effiduase, Dr Samuel Obeng Road, Effiduase, New-Juaben Municipal, Eastern Region, P.O. Box EF 3, Effiduase, Eastern Region, Ghana
  • Digital Address: EN-033-8445
  • General enquiries: info@3cghana.com

3C provides nurse-led, multidisciplinary home-based healthcare services, with the aim of supporting patients safely in their own homes and preventing unnecessary hospital admissions.

These Terms of Service ("Terms") govern your access to and use of the services described below, whether you are a patient, a family member/carer acting on a patient's behalf, or a partner organisation. By engaging our services, signing a care agreement with us, or using our website/app, you agree to be bound by these Terms.

If you do not agree to these Terms, please do not use our services.

2. Our Services

3C's services may include, depending on the individual care plan agreed with you:

  • Nurse-led home visits and clinical assessment
  • Multidisciplinary team input, including therapists (e.g. physiotherapy, occupational therapy) and carers
  • Care coordination and liaison with your General Practitioner (GP) or other treating doctor
  • Support for safe discharge from hospital into the community, in partnership with acute hospitals and other facilities
  • Ongoing monitoring aimed at identifying early signs of deterioration
  • Referral back to hospital, a GP, or emergency services where clinical deterioration is identified or admission is indicated
  • Administrative and care coordination support

Services are provided subject to clinical assessment. Not every service is suitable for every patient, and 3C reserves the right to decline, suspend, or discontinue care where, in the clinical judgement of our team, home-based care is no longer safe or appropriate.

2.1 Not an Emergency Service

3C is not an emergency medical service. In a medical emergency, patients, families, and carers must contact the relevant national emergency number or attend the nearest emergency department directly. 3C staff will assist with arranging emergency transfer where they are present and it is safe to do so, but 3C does not guarantee immediate response outside of scheduled visits or agreed on-call arrangements.

3. Eligibility and Onboarding

  • Services are available to individuals (or their authorised representatives) who complete our referral/intake process and are accepted following clinical assessment.
  • Referrals may come directly from patients or families, from hospitals, GPs, or other healthcare providers with whom we hold a service agreement.
  • We will ask for relevant medical history, current treatment information, and consent to share information with relevant treating professionals, as described in our Privacy Policy.
  • Acceptance onto our service is at our clinical discretion and may be reviewed at any time based on the patient's changing needs.

4. Patient and Carer Responsibilities

To help us provide safe care, patients and/or their representatives agree to:

  • Provide accurate and up-to-date medical, contact, and next-of-kin information
  • Inform us promptly of any change in condition, medication, or circumstances relevant to care
  • Provide a safe environment in the home for staff to deliver care (e.g. access, hygiene, safety of pets)
  • Treat 3C staff with courtesy and respect; we reserve the right to suspend visits where staff safety cannot be reasonably assured
  • Follow agreed care plans and raise concerns about the plan with the care team rather than unilaterally deviating from clinical advice without discussion
  • Understand that home-based care has inherent limitations compared to a hospital setting, and that clinical judgement may result in recommendation for hospital admission at any time

5. Consent to Care and Information Sharing

Before care begins, we will seek your (or your legally authorised representative's) informed consent to:

  • The proposed care plan
  • Assessment, treatment, and monitoring by our multidisciplinary team
  • Sharing relevant patient information with treating partners (such as referring hospitals, GPs, pharmacies, and specialist services) only where a contractual medical/data-sharing agreement is in place with that partner, as set out in our Privacy Policy
  • Being contacted regarding appointments, care updates, and, where relevant, service feedback

You may withdraw consent to specific aspects of care at any time, subject to the clinical risks of doing so being explained to you. Withdrawal of consent to essential monitoring or information-sharing with treating clinicians may affect our ability to safely continue providing services.

6. Partner Organisations

3C works with acute hospitals, GPs, allied health providers, and other medical/social care partners to facilitate safe discharge and continuity of care. Any such partnership is governed by a written contractual agreement covering, among other things, the scope of collaboration, clinical responsibilities, and permitted use of patient information. Patient information is not shared with a partner organisation unless such an agreement is in place.

3C is not responsible for the acts or omissions of independent third-party providers (such as a hospital or GP practice) except to the extent that 3C's own staff or systems were at fault.

7. Fees and Payment

  • Fees for services, where applicable, will be set out in a separate fee schedule or care agreement provided to you before care begins.
  • We will notify you in advance of any change to fees for ongoing care.

8. Limitation of Liability

  • 3C will provide services with reasonable skill, care, and in accordance with applicable professional and clinical standards in Ghana.
  • To the maximum extent permitted by law, 3C's liability for any claim arising from the services is limited to the value of fees paid for the specific service giving rise to the claim, except where liability cannot lawfully be excluded or limited (including liability for death or personal injury caused by negligence, or for fraud).
  • 3C is not liable for outcomes resulting from a patient's or representative's failure to follow the agreed care plan, failure to disclose relevant medical information, or delay in seeking emergency care.
  • Nothing in these Terms limits or excludes any liability that cannot be limited or excluded under Ghanaian law.

9. Insurance and Indemnity

3C maintains professional indemnity and public liability insurance appropriate to a home-based healthcare provider.

10. Suspension and Termination of Services

We may suspend or end services, with reasonable notice where clinically appropriate (or immediately in an emergency or safety situation), where:

  • Continuing home-based care is no longer clinically safe or appropriate
  • Fees remain unpaid after reasonable notice (where applicable)
  • The patient, family, or carer's conduct puts staff safety at risk
  • The patient or representative requests that care end

You may end services at any time by notifying us, subject to any notice period in your individual care agreement and to a clinical handover discussion for continuity of care.

11. Complaints

If you are unhappy with any aspect of our service, please contact us at info@3cghana.com. We aim to acknowledge complaints within 10 business days and will investigate in line with our internal complaints procedure. Nothing in this clause affects your right to raise concerns with the relevant Ghanaian health regulatory body.

12. Changes to These Terms

We may update these Terms from time to time, for example to reflect changes in the law, our services, or our geographic coverage as we expand. We will notify active patients/partners of material changes. Continued use of our services after changes take effect constitutes acceptance of the updated Terms.

13. Future Expansion

3C currently operates in Ghana. Should we expand into other jurisdictions, we will issue jurisdiction-specific terms as required by local law, and these Terms will continue to apply to services delivered in Ghana unless expressly stated otherwise.

14. Governing Law and Jurisdiction

These Terms are governed by the laws of the Republic of Ghana. Any dispute arising from these Terms or our services shall be subject to the exclusive jurisdiction of the courts of Ghana, without prejudice to any statutory right to raise concerns with a regulatory body.

4. Who We Share Your Data With

We only share patient information with third parties in the following circumstances:

Should we expand internationally, or use service providers based outside Ghana (e.g. cloud hosting), we will ensure any cross-border transfer of personal data complies with the Data Protection Act, 2012 (Act 843) and, where applicable, the laws of the destination jurisdiction — including using appropriate safeguards such as data processing agreements.
